Lamar Polk (he, him, his) has over 14 years of advanced-practice experience in behavioral health, public health, and health care. He brings specialized knowledge and skill in evidence-informed behavioral health practice; behavioral health integration and system redesign; recovery-oriented systems of care (ROSC); culturally informed, intersectional engagement; holistic care coordination and care transitions; racial equity and social justice; social determinants of health; and public-community partnerships. Mr. Polk currently serves as a learning coach for TAC’s Systems Alignment Innovation Hub, funded by the Robert Wood Johnson Foundation to help practice-based organizations in low-income and BIPOC communities to expand and utilize the evidence base for solutions that improve health outcomes and reduce racial and social disparities.

Prior to joining TAC, Mr. Polk worked for Massachusetts’ Single State Authority for Substance Use as the Assistant Director for Priority Populations, where he developed statewide efforts to better serve priority populations affected by substance use. This included developing a $2.3 million statewide pilot program for Black and Latino men at risk of fatal overdose upon reentry from incarceration. Mr. Polk is a Certified Advanced Social Work Case Manager (C-ASWCM) through the National Association of Social Workers and a Certified Case Manager (CCM) through the Commission for Case Manager Certification. He is currently pursuing his Doctorate of Public Health with a concentration in Health Equity and Social Justice at Johns Hopkins University.
