Three types of grants were awarded:

- Three states (Colorado, New York, & Washington State) will receive Model Pretesting Awards to continue developing their plans, with the goal of beginning implementation within 6 months
- Sixteen states (California, Connecticut, Delaware, Hawaii, Idaho, Illinois, Iowa, Maryland, Michigan, New Hampshire, Ohio, Pennsylvania, Rhode Island, Tennessee, Texas, & Utah) will receive Model Design Awards to create innovative State Health Care Innovation Plans.
